Replace V5 with a known good tube. Next, unplug the footswitch from the back panel to make sure the switch is not bad. Anything else will be internal and better left to a tech. Most likely it will be one of these two items though.

Thanks man! Was in the middle of a gig. V5 was frosted over white. Put in. Afresh 12ax7 and back in business!

Thanks for the info Doc. I just noticed that the tube was a 12ax7WA (i think). Is a regular 12ax7 suitable?

Yes that is Sovtek 12AX7 WB, which is typical lower gain 12AX7. In that circuit it is used as an oscillator, so any 12AX7 will plug in a work just fine.
