I bought a Maz Sr 1 x 12 combo with a Dr Z 12" 8 Ohm extension cab. The back of the combo has two 4 Ohm output jacks. Is is correct to connect each speaker to the 4 Ohm outputs?

I'll let someone else with the knowhow answer, although I think I know the answer.
Curious about the speakers you're running and the impact you are getting from adding the extension vs only the 112. My Maz 38 has a Lead 80 and I thought I would exchange it out, but run a number of things through an extension and never found one that was any better. I liked balancing with the extension, but ultimately I'm mic'ing and sending my amp to the pa that way. I moved on from the extension because I never have the opportunity to move that much air to make running two 12" speakers worth it.

OK, I’m officially confused.
Something doesn’t sound right. Your combo should have 4, 8 and 16 ohm output jacks, not two 4 ohm jacks.
Just for a minute pretend that the extra cab you bought does not exist and re-confirm what speaker output jacks are actually on the back of the amp. Then check the speaker actually mounted in the combo and tell us what it is and what ohms it calls for. If you bought the amp new it’s probably plugged in to the correct jack but double check anyway.

The Maz Sr combo has a Celestion G12H 8 Ohm speaker, the extension cab has the same speaker at 8 Ohms. The back of the combo has 4, 8, 16 and another 4 Ohm speaker jack, added by Dr Z. Running the combo alone, I use the 8 Ohm output.

Running the combo alone, I use the 8 Ohm output. I would contact Don at repairs@drzamps.com and give him the serial number and see if he can find some record of it. The regular 4/8/16 ohm transformer taps are meant to be used one at a time. "maybe" that extra 4 ohm jack is in parallel to the existing 4 ohm tap in which case you would move the internal speaker to the 4 ohm jack and plug in the external 8 ohm cab into the other 4 ohm jack. Don't quote me on that though, I'm completely guessing. If Don can't find a record of the mod, maybe pull the chassis out and send him a picture of how it's wired. You could probably figure it out with a continuity meter too, but please check with Don first.

And what is the Serial Number of this used MAZ 38.

Yes, but I think it would be safer just to go to PCNS and get a Y-cable and plug into the actual factory 4 ohm output. Safer yet would be to send it in to the mothership and have them put it back correct as delivered stock. Who knows what might have been hoo-dooed up when such a non-factory mod was made? I really can’t see any reason to have done it in the first place when a simple Y-cable would have done the trick.

+1 The Pro Cables N Sound Happy Cab and cables are legit. I've got a few sets for this exact purpose. Stuff is build like a tank! Works great!
